I've also received the same instructions about cath, subcu, exam, etc., but can't remember the source. It might have been a past company's specifications. I know that these words MUST BE EXPANDED (per BOS) in diagnoses, operation titles, etc. (same rules as abbreviations). This might be a new HIPPA rule also which came out after the second BOS was printed. I'll keep thinking. But he's not to be flamed. I've heard the same rules and don't mind using them because it looks more professional and gives me a higher character count.

ASSESSMENT: We discussed options for Jeff. At this point, a month after the injury, an MRI, potential surgical interventions, etc., ultimately doing as well as he is, he is just going to keep it in mind and be careful with his lifting. He drives a forklift, apparently on a regular basis, but we are going to forego further work up at this point. We also got int a discussion of routine health care maintenance. He will plan on complete physical in October, starting this year. I will put a standing order at lab at this point for him, for a UA basic metabolic fasting lipid panel and PSA. We will go from there.

verbatim # rule, begin sentence?
Subject: verbatim # rule, begin sentence?

On verbatim account, when doc dictates a number at the beginning of a sentence how do you handle it? For instance, "Forty-two-year-old woman...," I spell it out right? Also, another sentence begins with "4-0 Vicryl was also used..," what do I in this situation? If they are strict on verbatim, just go with number? 4-0?
