One patient could have multiple specimens, but they would examine and describe one specimen at a time. It has nothing to do with how many cells or anything. In fact, a nurse collecting the specimen often puts many pieces of tissue in one cup, as in a TURP specimen. The whole containerful may be called "the specimen."
