I believe you are looking for Calve-Legg-Perthes disease. There is an umlaut over the "Calve" but not sure it will take here.

The term I'm hearing sounds like "elegant"  On this report he is saying "elegant REM cycling."   Could the word really be elegant?  What does this mean?  TIA.

I could easily produce half a dozen "Books of Style" that contradict each other.

AAMT is not infallible. Their BOS provides a guideline, but it should not be used as a "bible".

do you need to know how to spell it, but also learn what it is that you are typing (and save yourself the embarassment of having one of your reports make the "laughing rounds" at the hospital or company that you're working for). :>)

Does this make sense to anyone?  Thanks for any help.

Will get EMGs. The patient has evidence of s/l palmarthrorheumatosis. He runs it together, so it may be palm (or palmar) arthrorheumatosis.

Any help is appreciated. :)

This is not an account where I have any editorial leeway.  The doctor states, "She was evaluated by an orthopedist, who kind of poo-poo'ed the patient's complaints and apparently did not do an x-ray exam."

Opinions on how to spell "poo-poo'ed?"

Thanks!

I haven't used one in a long time, but
Subject: I haven't used one in a long time, but

I would either call tech support for Sony, I was trying to google and troubleshoot that way, but couldn't find anything, or find an office repair shop in your area as some of them still fix those machines, that way you don't have to go out and buy one.  Or if you have work you have to do now, I have had to go out and get one in an emergency just to finish the work while I got mine repaired, then had a back-up machine.  They really don't make a good machine anymore and they break down frequently, in my opinion.  I don't know how long you have had yours or how many hours a day you're on it, but it might be time to replace it.  Sorry, I wish I had a clear-cut solution to tell you.  It's so frustrating when our equipment fails us and we have so much work to get done.

Subject: q. hour of sleep

I e-mailed AAMT with this same question about a month ago.  Their response to me was as follows:

"He may be dictating "hour of sleep" because he expects you to use the abbreviation h.s. However, this particular abbreviation is on the Dangerous Abbreviations List, and we are encouraged to transcribe this as either "nightly" or "at bedtime." If the dictator questions you about this, you can show him the appendix in your Book of Style or refer him to the Dangerous Abbreviations List under Patient Safety Goals on the JCAHO web site (www.jcaho.org)."
