I've always transcribed Augmentin 875 mg p.o. t.i.d. (or whatever) for,,,,sm

ten days, #20.  By putting the #sign modifier, it separates and makes clear the number of pills in prescrption, versus the mg or the times per day, etc.

a separate heading.  Another old timer here.  I don't know the original post, but I agree with separating out the arteries.

spelled it that way (I always left a blank in the report ~ he would always "write" it in when he got the report back). This was many, many years ago when I worked in-house. I honestly thought it was a "made-up" word as he was the only doctor who used it (I have never heard it used since), so I was surprised to hear of somebody else stumbling onto the same word (or something similar). By no means did I intend to mean that that was the "correct" spelling or that I used that word ~ I apologize if that is how my message was read. :)

When I google it, I'm finding both.  Cannot locate it in a Stedman's book.  Doctor spells with "a" but you can't always go by the spellings!

Would you use a roman numeral for Type I Aortic aneurysm or use Type one or type 1 ... I am thinking it is Type I but I am unsure. I am new to this all. Thanks for any help.

I have had that happen to me before, and I was told by QA to not move things around in the report, maybe that is the same for you, but if I were typing this report, I would type it in the order as is dictated, even if that means typing it under the heading PLAN.  Also, his vitals were probably taken at the end of the exam as they were repeated, so it would make sense to type them again under that heading.  Hope that helps!

Yes, it used to be Type II, but clarity is more important, so it was changed. Either would be acceptable in transcription, however.

It has always been Merchant view, as far as I know.  All my Sted refs have Mechant view. Though I do not do BOS, I typed ortho for 4 years in a railroad hospital with lots and lots of injuries/surgeries etc.
