which doesn't make it right.

I avoid ever doing this, however: 1% Xylocaine was infiltrated. (I saw this done recently and this is what happens when the secretary in the office types up normals and they don't get an MT to do it). I will flip them for this: Xylocaine 1% was infiltrated.

Doing this: CT scan of the abdomen and pelvis was ordered...yup I leave it that way.
