but before the signature. Makes it easier. We typed our reports on sticky paper and that way it did not get separated from the main report etc. But I did not do BOS. nm

only psych reference book I had. It was the old Stedman's one that had psych and neuro mixed together. HATED that book, LOL.

Not only is psych boring, but a lot of it is actually drug rehab, and those patients' problems are 95% self-inflicted out of pure foolishness.

I have a dictator today, she is saying, "Review of systems: s/l perinegative except for ...." and, "exam is s/l perinormal with EKG done today showing normal sinus rhythm"


Anyone hear of this "peri"normal "peri"negative and is this how to spell it? Can't find anything conclusive on google in any letter combination I can come up with.

as no one else here seems to know how to do that.

I also wanted to see what would happen once posted, and you helped underscore my point. We can be good MTs or we can continue to be mediocre and then need to shut up about work going offshore. It is one thing to know when to use disc and when to use disk and another to give the client what they want, BUT to adopt an attitude of not knowing when it is proper to use these words, we diminished our value.

Try looking up articular d. in Stedman's Dictionary and see which d. it instructs you to use. If you use disc for ortho, you might be using the wrong one.

Again, give the client what it wants BUT please know the difference and when people ask questions here, do not enable them. Educate them. Know how to be a proper MT, even if your client asks you to transcribe improperly.

No matter how I transcribe it, QA feels it is incorrect, but Allen Medical shows Yellofin Elite Stirrups and has a Yellowfin stirrup O/R table for ordering and prices.  I'd been taught long ago that this was a commonly misunderstood term and often dictated incorrectly by doctors and that the correct term was Yellofins -- no matter how they said it.  This term is documented on another reputable MT site.  It was so much easier when I felt that was the correct term.  Arghhhh!

urethrovesical junction was neither hypermobile, patchulose, nor fibrotic...

I just cannot seem to get a grasp of this guy today. Pt is seen for congestive heart failure. Here are the sentences leading up to my problem:


We will check a BMP today to reevaluate renal function and, if he does have a markedly elevated creatinine, we may not be able to do this (referring to 64-slice CT angiography). I am hopeful that, with ___ and renal protection, we can get by with this. We will also plan on checking a TSH and baseline blood work, including a CBC and BNP for outpatient baseline.     


Sounds like mucavest. I then thought of Mucomyst, but would like to know if anyone agrees because it does not make sense to me.

ALT L with start numbered list lining them up and ALT L will shut it off.

Or start typing without the numbers, returning as needed and then block listings and ALT L.

Or, type 1, tab and type and when you return for #1 it will give you the number. If it indents, back up to the beginning of one of #1 sentences and SHIFT TAB to move it back the margin.

There is a complex closure of one layer of deep s/l *sutures or stuctures using 5-0 chromic gut, and then a superficial running, locking stitch using 5-0 Ethilon. The area is hemostatic. Sterile dressing is applied.

*It actually sounds more like sutures, but would that make sense? Thanks for any help.

At the risk of sounding dumb, I need to ask this as it happens quite often.  Math was always my worst subject in school... Dictation  states pt is on a half mg of something.  For example "spironolactone 12 and a half mg".  I know they want me to type it as a decimal rather than writing out the words.  Is the proper way to write this 12.5?  (Does .5 equal a half mg?)  Thanks!
