Thanks for all the input.  It is helpful to get opinions from other MTs with more experience. I really love coming here for advice and trying to give some if I can.

If you check Dorland's, you will find that both are acceptable spellings for the instrument.

I then took a 2-mm punch biopsy in order to get a tissue sample culture then completed the process of debriding the small wound with a 3-mm curette or curet?  Stedmans has both as same thing?

Dorland's lists curet as the preferred spelling.  Interesting, huh?  Neither is wrong.  Don't sweat the small stuff.

If you look up both curet and curette in medical dictionary, curette has a definition of a spoon shaped surgical instrument, etc., but curet is only listed as a variant of curette.  I use curette as a noun or verb and stay away from the variant curet, especially on picky accounts.  FYI.

Medication dosages, QA marking wrong both ways!

Good afternoon everyone.  I'm in need of some second opinions please.  When typing medication dosages, I have always transcribed them the same way for the last 12-1/2 years.  For example, Lortab 5 mg q.4h. p.r.n. pain. 


I have worked for the same company for a little over two years now and have never had QA say anything about this until last month, when I got a QA audit marking it off and changing it to Lortab 5 mg q. 4h. p.r.n. (with a space between the q. and the 4h.)  I thought okay, maybe I've been transcribing it incorrectly all lthis time and a new QA person picked up on it.  It happens, no big deal. 


So, I started transcribing it their way with the space, as in q. 4h.  and didn't think anything more about it.  Yesterday, I get another audit.  This time, I get dinged for typing it the way they wanted it the last audit q. 4h. with the space with a note stating it should be q.4h. with no space (the way I had been typing it). 


Now, I know they have been messing with all of the MTs big time lately, making changes daily from everything to hours, pay, policies, and procedures for vacation, days off, etc.  You name it, they've been doing it, and I really think they are trying to either find a reason to fire some of us or drive us to the point of insanity until we quit, but despite the games, I still want to make sure I'm doing my job correctly.  If I've been doing something wrong, I would like to correct it ASAP, except now I have no idea which one is correct. 


Sorry for the long, drawn out post, but I'm totally confused now and beyond frustrated.  Now, I'm questioning myself on everything, and I have no idea which way is correct as far the space or no space with the medication dosages.  Thank you for taking the time to read my question and rant.  I would appreciate any and all opinions on how you all transcribe this!

What kind of surgery is it? If he used "lap" laparotomy pads may be the second one. They usually say sponge, needle and instrument counts. Will keep thinking on this one. Type of surgery would help.

this is given, or any other context, so I have to wonder if this isn't a problem list perhaps giving more than one diagnosis/problem here (as in (1) portosystemic shunt and (2) nephrolithiasis)...

Peripheral -s/l orb or oral- sinuses were intubated to their apices and were normal.


Specimen was sent -s/l informal- and for pathology.

Next, fluoroscopy was used to place new screws at the L2-3 segment.  Starting points were identified at the confluence of the midpoint of the transverse process, the **manloid** process, and the pars interarticularis



He really mumbles through this part, so I am just not sure, but I have not heard this term used before. Thanks!

FINDINGS:  Epidurogram revealed normal (s/l-cephaloquadet-) contrast flow outlining the right L5 nerve root sleeve in the epidural space with good flow achieved medial to the right L5 pedicle.  The patient did experience some concordant L5 symptoms on the right side during the injection.  Vital signs remained stable throughout.

Hi all! I am doing an OP for a lumbar spine surgeon, but this is just for an I&D. He says the area was probed with "sucker tips" and was found to be subfascial. I am not too comfortable with what I am finding online for this. Has anyone heard this before? Thanks!
