Subject: I wish it were so, but....
I'd personally prefer to see alk phos expanded at all times to alkaline phosphatase, but the reality these days is that whether or not one expands it is determined by your service and/or client ... many prefer you /not/ expand it (both in the interest of verbatim transcription, as well as saving a few pennies here and there). So, depends upon your style guidelines/site notes.
