I agree with the above.  I would do it this way...

CLINICAL IMPRESSION:
1. Fever.
2. Bronchitis, rule out a right pneumonia.
3. Chronic lymphocytic leukemia.
4. Atrial fibrillation, rule out arrhythmia.
