A bolus is not a drip rate.  They mean two different things.  KVO is an old-fashioned way of saying TKO, but they mean the same thing.  Keep Vein Open = To Keep Open.  It is not a set rate of ml/hr however, it is just a slow drip, just enough fluid to keep access to the vein patent.  A bolus means to push fluid or meds at a rapid rate but again it's not a specific drip rate.
