Could be that he is reading what the patient wrote. Happens to me all the time.

I'm auditing... the doctor clearly says, "Hypothyroidism.  We will check thyroid-stimulating hormone at this visit and continue the patient on Levoril 50 mcg by mouth daily."  But the MT typed Levoxyl.  When I do a google search for Levoril, I can't find anything and it's not listed as a generic under Levoxyl.  I'm confused.  Anyone have any info on this??
