Is it possible they are simply repeating exacerbating? Like "exacerbating the...exacerbating anal pain"? Maybe they wanted to get rid of the 'the'.

Alternatively, how about excruciating?

In earlier days I was assured that guide wire was a generic term and Guidewire was a brand name, but I sure see it in a lot of sources where it is not capped.

My best guess is yes, cap it, or check with your QA department if you have one.

I bought the American Medical Association style guide and it, too, reflects these changes. AAMT BOS is just a trickle down from the AMA, Chicago Manual of Style, and the other many sources of which it is comprised. The changes are industry-wide, not just for us, but our work is what is permanent on the patients' charts so we have to pay particular attention to them.
