Technically it's pulsatile lavage, but pulse lavage is standard slang used all the time.

I don't really have any suggestions for your missing words, but I do have a few general suggestions. 


Are the terms in parenthesis the ones you don't know?  If they are, put s/l in front of it (s/l= sounds like), and then do your best to type what you are hearing phonetically.  Sometimes it also helps people figure out the missing words if you can share as much about the physical condition of the patient that you know.

It was really unclear.

It may have been trapezius, but I looked at it from every angle I could, thinking he may have been saying TM - for teres minor --

You are probably right and the doctor is misprouncing the word. I still hear a K and X sound so I shall leave a blank just in case being that I do not have a firm diagnosis to match it up with. Thanks for your help!

Here's the way I would do this:

** I took out the word ""with"" because it sounds awkward and the word ""preceding"" I believe should be ""proceeding"", and then with the 2 commas.

I would change the units of insulin to 10 and 20 instead of using ten and twenty. Maybe doc is saying this instead... He has been taking 10 units of Lantus. Since then sugar levels were 90 to 100.

I listened again and it doesn't sound like gritty. I think it's "cri". I found a couple reports with this same procedure and that word used. I also found the first one, which is "transcervically". Thank you for the suggestion though! :)
