something like "abdominal pain" and "status post surgery."  Then, you would really have to take into account what is going on.  "Abdominal pain, status post surgery" would mean the patient had surgery done because of the abdominal pain.  "Abdominal pain status post surgery" would mean the patient had abdominal pain after surgery.
