PHYSICAL EXAM:  On exam, temperature 96.  Weight 49 pounds.  Running about the room.  Cooperative.  Conjunctivae normal.  TM on the right 30 percent or greater perforation is (s/l dredged) in the inferior posterior portion of the TM.  There was no debris.  There is no purulence.  The left reveals a slightly thickened TM towards the periphery but very mobile and normal-appearing essentially.  The EACs are clean and clear.  Nares patent.  Oral normal.  Neck normal.  Chest clear. 


 


ASSESSMENT/PLAN


Large inferior posterior perforation on the right.

Question about eye exam...when everting lids on eye exam (sm)

It is EVERT, not AVERT, right? I have always typed it with an E but I have a PA who keeps insisting it is A.  I always thought Avert applied to gaze, as if to look away.  Just want to make sure everting the eyelid is the right term.  TIA.

it really pays to subscribe to the MPR, monthly prescribing reference, it is really good. relatively cheap and specially priced for the transcriptionist.  lists all new drugs every month    I can't live withou it
