I absolutely HATE leaving blanks in a report and will do so only when I have exhausted all other avenues - listening again and again, asking here, looking things up in books and online, etc.
Do you have a QA department or a supervisor to whom you can pend this document? Perhaps than can offer you some assistance before sending it through with many blanks.
Sometimes, because of poor audio quality or because of poor dictator quality :) you canot help but leave blanks. But I try to avoid it at all costs. That's just me.
