Thank you. It is CNS. I went back and relistened. I am losing it here today,too many hours trying to help catch up, the brain must be slowing down, lol. Your help is much appreciated.

Thanks, but my physician mumbles it so fast that I cannot make out if it is "hold and call" or "hold, on call."

Is there something like "hold and call" for any test like an x-ray or CAT scan, that needs to be done urgently on a patient before the patient leaves the office?

He said the pupil was not dilated enough during a cataract surgery, so he used a ? ring was used to hold the pupil open.  I have done hundreds, if not thousands of report for this ophthalmologist, but my whole day has been like this!

say your s/l really fast it "sounds" like drug-eluting stent..? lol worth a try anyway, couldn't find anything else.
Outpatient hold admission versus inpatient

Please read this with a little grain of salt, since I don't have enough info to be sure in this case, but this has happened to me before. Most hospitals and most insurance companies have a "24 hour hold bed" policy of some sort. The patient is considered an outpatient, (and charged the lesser "hold bed" charge) even though admitted to the hospital, for the first 24 hours, or 23 hours and 59 minutes. Maybe this doc is saying that the EKG was done at that time in the admission - hour #23, minute #____. Again, use caution, and if this is the case, the doctor sure SHOULD have said "at 23 hours X minutes after" admission or clarified it somehow. But this is just food for thought, since I have had docs dictate using the number of hours since the patient was admitted, though I'm happy to say they the docs always made it known that that's the way they were counting down the hold bed time.

Perhaps guards as in bumps to pad or hold the patient during surgery.

The doctor is saying the "rest of the review of systems are negative." Shouldn't it be "rest is negative."  ?? Or is rest considered pleural?  Help me, I can't think today!!


You know what's bad? I have heard so much ESL butchering of grammar that improper grammar is starting to sound correct to me.

Colorectal today...

She has a history of rectal prolapse and had ____water placed and there was some question if the ____water perforated her vagina creating a rectovaginal fistula.


It sounds like he is saying "tertiary" but that does not make sense.  I read somewhere that warm water is sometimes used to reduce a prolapse but I'm not coming up with a word for "warm" even in the thesaurus that sounds like that.

In ENT surgery.. the graft was meshed S/L  "to 1-1/2 to 1 and placed over the muscle.  This 1-1/2 to 1.... not sure how to type.. is it a ratio? TIA
