Dumb question...
Subject: Dumb question...

At the risk of sounding dumb, I need to ask this as it happens quite often.  Math was always my worst subject in school... Dictation  states pt is on a half mg of something.  For example "spironolactone 12 and a half mg".  I know they want me to type it as a decimal rather than writing out the words.  Is the proper way to write this 12.5?  (Does .5 equal a half mg?)  Thanks!

I know this sounds like a dumb question but here goes...

LEFT ANKLE PAIN WITH INSTABILITY

Evaluation of the ankle shows no obvious deformity. There is no real effusion of his S/L knee-ankle joint today. He is tender to palpation...

This may sound like a dumb question but it is important for the setup of the report.

First the Dr. says knee aspirit ( I assume knee aspiration) Also the main question is would a knee aspiration be considered a surgery? TIA.

I was wondering if this is one diagnosis or 2 diagnoses.
1. RIGHT SHOULDER IMPINGEMENT SYNDROME
2. ROTATOR CUFF TEAR

The only reason I ask is because I was just released from QA and have never been so nervous in my life about making a mistake. My security blanket is gone..lol. I may be asking a lot of dumb questions for a while until I feel secure again..lol.

The patient is now 1 month post fracture of her left ulna *Dr says left ulnar* should be ulna? not ulnar

also..

There is pain at the superior vetebral border of her right scapular *Dr. says right scapular* should it be scapula? not scapular.
TIA.

Since he didn't say it as you did, does saying "larges" constitute shorthand in doctor speaking?

every day.  I didn't mean to sound so desperate with my post, but she was a very difficult dictator and I was already extremely frustrated by the time I got to that point in the dictation.  One of those that can't make a complete sentence and keeps changing their mind about what they are going to say.  Anyway, thanks to all of you for your help.  I won't be forgetting that word, I guarantee you that.

The patient will be seen for followup in a week.

Followup one week.

The patient came in for follow up of her acne. (I believe this can be hyphenated to follow-up.)

The patient will be seen for followup care in a month. (This, too, some MTs hyphenate to follow-up but I do not.)

The BOS has this very issue - Book of Style by the AAMT 2nd edition  - discussed in it.

Subject: We learn every day -- don't be afraid to ask your QA why? ..see msg

I'v been doing this for quite a few years, and last night had a report where I heard "Shoreman kyphosis," researched and found quite a few websites spelled this way, didn't really check to see source of websites, just that there were a LOT of them. (slap me with a wet noodle, I was tired and brain-dead because I know better)

Anyway, QA send me an email this morning with correct spelling, "Scheuermann kyphosis." citing Dorlands online.

You can bet I created a shorthand entry for that spelling immediately, and of course, sent of an email to my QA person letting her know how much I appreciated the feedback -- I never have questioned my research without the feedback!

Have a great day.
