Headings

Newbie here.  If a doc dictates a laboratory data and lists lab values, then talks about x-rays and ekgs, do you include that in the lab section or make a separate heading for x-rays and ekgs (what would you title that section?).  Thanks for any info.

normal said.  For example, if it says "breasts are soft and pendulous", you might want to leave that out.  Men generally don't have pendulous breasts, not even ones with gynecomastia.  If it simply refers to the fact that there are no masses or nipple discharge, I would probably leave it in.


If you're still unsure, you can always flag it and find out for future reference.

I am transcribing a History & Physical Examination Report  (Neurology)


The headings confuse me terribly.  What would the heading be for the following: 


Orientation and language are normal fundi with sharp disks bilaterally.


I have the following: 


NEUROLOGIC:   Patient walks to the right occasionally. She has trouble getting started because of discomfort. She can walk on heels and toes briefly. However, on tandem walk she really has difficulty with that. She does not line up her toes easily and loses her balance very easily with that.  (Is that the correct heading?)


Also, does anyone have a good website for determining headings?

Sorry, I don't have a quick answer about your headings, but I wanted to let you know that your first sentence should be 2 sentences: Orientation and language are normal. Fundi with sharp disks bilaterally. (Fundi and disks/discs refer to the eyes, nothing to do with orientation or language.) Good luck with the headings - I still get confused sometimes on them too if I don't have a specific format to follow.
If QA has no recommendations, you don't have to have headings in there at all. Just do the report

Spleen: The spleen has a thin fibrous capsule. Malpighian bodies are indistinct the sinusoids are moderately widened.



Liver: Sections of the liver show a thin fibrous capsule. Multiple small fatty vacuoles are noted in the cytoplasm. Portal spaces are fairly small in size. There is mild widening of the sinusoids in the central zones.



Kidneys: The kidneys have a thin capsule. The cortices that show glomeruli of normal appearance. The tubules are not remarkable.



Interstitial tissue showed no note worthy change. The pelves are not remarkable.


 


In this instance.. does the interstitial tissue and pelves need their own headings.. or is this correct?? Thanks

This is for a psych. consult.  After saying "New paragraph", he says "Review of past medical records." and then launches into: Doctor diagnosed tachycardia..etc.


I haven't seen a consult. with a heading, main or sub, with Review of Past Medical Records.  Past Medical History, yes, but not the other.  Any ideas?

One of the tests for meningitis is to bend your chin to your chest or to your shoulders. So if the pateint can to this, they have no meningeal signs or meningismus.

Newbie here. I'm typing an H&P and under instruction to supply appropriate headings & subheadings if they are not dictated, esp. in an H&P.

Under the Physical Examination, the Dr. dictated most headings, but under ABDOMEN, she continued with what I think is more of a musculoskeletal exam:

ABDOMEN: His abdomen is rotund, nontender, nondistended, and soft. He has no costovertebral angle (CVA) tenderness to percussion. He has no thoracic tenderness to palpation. His pain begins at approximately L2 down to his sacrum, with pain greater down his right gluteus versus his left, but does have pain. He has slow movement and grimaces with any movement. He is able to stand. Decreased range of motion secondary to pain. Straight-leg reflex painful in both legs. Reflexes are diminished bilaterally.

Here is my question: I cannot find a precedent for adding the MUSCULOSKELETAL subheading. Yet beginning from "His pain" and following is not really under an ABDOMEN exam, is it? At the very least, I feel I should start a new paragraph, but then my format is messed up b/c everything else has a subheading!

Needless to say, it stresses me out a bit. Any advice is definitely appreciated!
