Strictures are usually dilated, not broken up.

in the past medical history sound like:  "Leptoscopy, levogram" November 2005 revealing possibly a left ureteroileal stricture. 


I can definately hear the "scopy" and "gram" but cannot find these words fitting the sentence at all...any ideas on either word...This is the full sentence...

causing difficulty that can be dealt with, then that would improve his nutritional status. This is why AAMT is now advising us to transcribe as dictated. So difficult to interpret these guys sometimes, but we get the gist of what they mean.

Usually it's a GI doc to deal with this, but maybe there is something in the throat of concern, or a swallowing dysfunction. I'd flag it for your QA and say "transcribed as dictated" and let QA handle it.

improve a patient's nutritional status., but rather do the complete opposite. I think the physician meant to dictate "GI" physician since he/she mentioned the dysphagia in the dictation, which is also a common place for strictures or lesions to develop.

sounds like the radiation caused either a cutaneous burn/erosion or an internal scar contracture so was stopped. Could it be "of 7 inches" or "of several inches"? Unfortunately very common side effect of rectal cancer radiation.
