Doctor dictates a nickname for the recipient of a letter, but the full name is listed in her contact info. Non-verbatim. Should I type it as dictated or change it to the more formal, full name?

I know of the zithromax z-pack but this doctor is clearly dictating T pak. Is this a new thing?

Thanks for trying. After listening to it over and over several times I figured out that she was saying Zithromax to treat for possible pneumonia, she was just saying it extremely fast! Thanks again.
