He's probably making "Fistula-in-ano plural with the final "s". You could type either four fistula-in-anos or four-fistula-in-ano. Also, I believe that fistula (being a Latin-based word) is the flural form of fistla (same spelling).

It would depend on what type of report you are typing, but I would say that generally your example looks okay. The dictator, transcriber, and date information does not change so it does not need to be repeated. That's how I have been doing it for 20+ years, anyway.

When doctor adds an addendum as an afterthought, where do you type it? I have been putting it after the signature line in the original report. But does it need its own report?

Thanks

When it's a trademarked drug - I find the drug company's website - the actual maker (speller) of the drug...and THAT would be more correct than the books which are not always consistent.

On his hearing test, the patient could not hear frequencies at 1000-2000-4000 with his right ear at 40 decibels.  He could not hear 1000 with his left ear at 40 decibels.  However, during the whisper test, he was able to hear three items in each ear from at least 7 feet away.

I have no idea how to type the 1000 2000 4000.  This is on a physical exam.  Thanks for any help!!

I would do it this way...

CLINICAL IMPRESSION:
1. Fever.
2. Bronchitis.  Rule out a right pneumonia.
3. Chronic lymphocytic leukemia.
4. Atrial fibrillation.  Rule out arrhythmia.

The reason I do it that way is because way before, we used to line Rule out under let's say Atrial fibrillation, yielding 2 lines.  It used to be done that way back in the early 90s.  Now it is not done that way.  So, I give the impression which would be atrial fibrillation and begin a new sentence....because they have to rule out an arrhythmia.  I don't think it is etched in stone, so either/or would be fine.  *S*

A headband for mounting a luminaire is formed of a perspiration-absorbent cloth strip, has hook-and-clasp engagement, and includes a luminaire mount.
