ENT polyp removal surgery from vocal fold. sm for sentence pls. s/l formal surgical fashion.
Subject: ENT polyp removal surgery from vocal fold. sm for sentence pls. s/l formal surgical fashion.

Using formal surgical technique, complete excision of this lesion was performed while taking care to protect as much normal epithelium and underlying superficial lamina propria as possible

The FORMAL could also sound a little bit like funnel or fur-nel??  I don't think formal is right, but not sure what I am hearing.
