test, they have the patient stick their thumb in the palm and close fingers over it and bend hand, so in essence you are "clasping" the thumb, so not sure if he is just saying the same thing when saying both tests were negative.  Just a guess.  Sorry can't be of more help.

It seems your patient either had a replacement or fracture earlier (is there arthritis in the hx?) I often hear lytic lesions or lytic lines in the areas you mention on patients with arthritis/oseoarthritis. Lysis is the process of destruction of something (in this case, bone), which seems to have been ruled out by these x-rays.

because too many times when he uses the term PFC there is no other indication of a joint replacement system having been done or anticipated, so I still have my doubts that that is really what he means by these initials.  Thanks for your help.

mack is a flexible word as a verb; it can be used in pretty much any situation and its meaning will be understood.
Thus, in your case, maybe the dictator chose the word mack as slang for height?.....because 23.5 cm is equal to 9.25 inches. I used this as an example, as I believe you were referring to dietary in a Parkinson patient?....Wish I could assist more.

exactly what she is referring to before making the decision to transcribe bilateral.  She may mean bimalleolar (ankle) edema.  If that were the case, bilateral ankle edema is very different than bimalleolar edema, which would mean 1 foot versus 2.  If you can leave it blank and get a response back, that is the best idea.
