with C. difficile colitis as well - CD/VRE...also, resolved to the NVRA just doesn't make grammatical sense - you sure it's not "The C. difficile was treated with Flagyl, but resulted in VRE, then treated with Zyvox?" GOOD LUCK!

Since there are so many different transcription platforms, you can't always program in the 1/2 symbol and typing it like that - 1/2 - is clumsy and can be unclear when read. The guidelines at my last in-house job were to convert it to decimal if possible.
