Okay, I hear "cart" and the later "hart" but I still can't figure out what it would be that would make sense - its can't be heart, could it? Any ideas?

Examination of her ear shows a large clot in her external auditory canal. I did flush her ear there. I removed a large clot that was __________ s/l small and (swollen-out-) bleeding from the spots where it adhered to the canal wall.


It sounds like small but can't be because he calls it a large clot.
