My thought is this: 


The thing that is causing a blockage is obstructive.  The area/pasage that is blocked is obstructed.  To use your context in an example:  if a tonsil is swollen and blocks the pharynx, then the the tonsil is obstructive and the pharynx is obstructed.  One relevant permutation of this that I can think of would be when describing a situation where the signs and symptoms of a blockage are apparent, but no one is yet sure what is going on...That is often referred to as  an obstructive picture.
