If dictated is June 4th, I have always typed June 4.  The AAMT BOS is not specific on this except to say type June 4, 2008 if you are sure of the year.  I was taught never to type dates as ordinals.. what do you all think?

it's just trimethoprim sulfate in Saunders, but when googling it, I found trimethoprim sulfa or trimethoprim/sulfa.

In the Sanders Pharmaceutical Drug Book 2008 it shows Benicar HCT, no slash

HCT is hydrochlorothiazide and when together, Benicar HCT means hydrocholorthiazide. It should not have to be spelled out and cannot be mistaken for hydrocortisone because there is no such thing as Benicar hydrocortisone. I would leave as is as your QA and others have stated. Hopefully this clears it up.

typed as Ƈ-year history', and another one wants it to be 'one-year history'. I think either is correct, though I would spell out the word 'one' if it's at the beginning of a sentence.
Mine wanted a hyphen in gastrocnemius, between the o and c no less.
It was only my second day, so I didn't say anything - maybe it was an accident.

But I think it is an anatomic landmark. I just thought if it was, it would be in one of my medical dictionaries or defined somewhere online more definitively!
