Stages are Roman numerals, TNM staging is Arabic, grades are Arabic. There are no commas used in the TNM staging per the BOS.

Also, I would transcribe February 2005 for the date, for whatever it's wroth.
They're usually very thin. I've only had them with the zeros in there like 0.035

Some accounts will not tell you but cc is a big no no on the Joint Commission "Do Not Use List" See site below.  Account I type for now, MTSO was/is not even aware.  They dictate cc but I change to ml and have not had a report come back yet.

h.s. is one of the abbreviations on the list. They say to substitute at bedtime.

Your account preferences take priority, however. I have one client who wants me to use h.s. and I do for them.
I believe that "q" is a prohibited abbreviation per JCAHO. I usually put "a day" or

JCAHO requires its members to follow an official "do not use list" and "cc" is being considered for possible future inclusion in the JCAHO "do not use list" to be replaced by "mL" because it can be misread as U or unit when poorly WRITTEN...typing it is not as problematic but I go ahead and change it to mL to encourage dictators to make the "mental shift" away from "cc" ...hope that helps! see this link http://en.wikipedia.org/wiki/List_of_medical_abbreviations

2.5 is 2-1/2 and 2.5 is the proper way to type it. It should be 2.5 mL in your sentence on your post. This is the rule for medications and recently they changed the way they want us to transcribe cc to mL, which was always the equivalent of each other and I don't know why they changed it (maybe they were bored). I understand that they think cc can be "miswritten" and "misread" as something else, but I think that should be directed at handwritten orders and documentation, not transcribed orders or documentation that is typed. The bottom line is they don't care what we think and we need to do it their way. I would never ever type 2.5 mL as 2-1/2 because it is not correct or professional to do so. Sometimes I think they just are bored and think of something to "fix."

Maybe you could narrow down which dx he is treating with this drug. With some of the mushmouth physicians I have, I would guess maybe oxycontin for the back pain.
