The cardia (or esophagogastric junction[1][2] or gastroesophageal junction[3][4][5][6]) is the anatomical term for the junction orifice of the stomach and the esophagus. At the cardia, the mucosa of the esophagus transitions into gastric mucosa.

I took out my huge A&P book(from a prenursing course), and could not find muscles with a similar sound. The muscles I found near the thoracic vertebrae were trapezius, levator scapulae, supraspinatus and infraspinatus.

i.e., the phrase isn't a conjunction and doesn't get treated the same way in a sentence. So your first choice is correct. I find that a lot of docs dictate w, x, y as well as z. I always change this to w, x and y as well as z.
