It may seem unlikely but....

300+ lph is possible on the right account.  Of course, the cpl is also a factor in that $25/hr also.  I know I am on a mid-difficulty account and can do 250-275 lph just about every day, but I also make more than 8 cpl, too (and I work for a smaller company also). 

As for getting oneself into a perfect situation/account/cpl, etc., I can say I started out at a lower cpl on one of the worst accounts I've ever had.  After about 3 months of struggling I decided I was selling myself short and refused to accept those conditions.  I went to my employer and asked to be put on a better account with better pay, and I got it.
