Send the reports back to the crappy dictator.  Garbage in, garbage out.  Services need to quit kowtowing to the accounts with the horrendous dictators.  When I first started MT in mid 1980s, any dictator who was unintelligible or rambling on with no focus was forced to re-dictate so the MTs could understand.  My HIM supervisor stood up for us.  This was at a teaching hospital with all kinds of ESLs and we turned out some pretty good dictators back then, great MTs and much more accurate reports. 


Clerks were also available to check demographics and out-of-the-area physician's names/addresses.  TRANSCRIPTION was deemed a priority, not wasting time looking up this or that demographic.  Things in this industry have sure gone downhill since.  We also had no need for QA people.  We were trained very well and given QA spot checks by the supervisor periodically.  Those were the good old days.
