Well I can say that this is probably

I worked at a hospital where the equipment itself for cardiac caths, EMGs, EEGs, etc. could literally spit out a typed report; however, the physicians preferred to dictate, plus the fact that it needed to be in the hospital computer database for all to access.  Hence, we transcribed in that database or re-typed the report from the actual equipment that performed the test.

The colonoscopy I had done was at a freestanding endoscopy center that has all of the new equipment and best of everything.  So, I guess that's why the report was ready in a flash.

I did ask when I went in if they could send me a copy and the girl kind of chuckled and said oh honey you'll have it before you leave today.  I thought to myself I am not waiting around for my report but that was not the case at all.  Pictures and all were included on this report.  It obviously was typed in by either a tech or the physician himself with the punctuatation errors but since I see stuff like all of the time either in newspapers, magazines, e-mails from superiors, etc., I really paid it no mind.
