Yes, I would definitely leave them a note and say I hit your dog and he seemed alright at the time but I just wanted you to be aware and have him checked out. Definetely under no circumstances would I leave your name especially if you are worried about retaliation. Just sign it a concern dog lover. I know no matter what you will feel horrible but do not be too hard on yourself. Accidents happen and that is why they call them accidents!!
