Docking for error..

I'm trying to remember what it was when I was there. I believe for 98% or lower, it is 5% of your check and up to 10% for 97% and below but this could have changed. I am glad to see that they have added some US QA editors. When I was there, I was only QA'd by Indian QA and that did not make me feel real comfortable since my check was going to be based on them QA-ing my work. Once I was docked 10% and that hurt but it was like losing a whole day of pay. Again, it sounds like they are really turning things around so best wishes if you opt to go with them.

Your error made me chuckle, which is, ya know, fitting for the error. Not a big deal, and no we ALL
make mistakes! Heavens! I remember starting out, I actually typed CABBAGE! Can you imagine?  Scary thing, though, is when you get an MT out 10 years and they type cabbage.  There are errors like yours, and there are major-absolutely-no-clue-of-our profession errors, which are mind boggling when you see them in print.  I've been on both sides, typing and proofing, and I never dreamed of the quantity and quality of errors some MTs make - HUGE errors.  That said, yes, we all make them, but its the ones who are unlearned and then arrogant when you point out the error - we have to, ya know, and its done kindly, so that they learn, and still that type of MT flips out and has a hissy! No win situation for both parties, really, as the MT really can't grow in the field if they aren't accountable to themselves even for their work.  I can't give more specific examples, as I could potentially hurt some MTs who would recognize their errors, but I assure you, some are unbelievable.

Docking your pay for ....
sending a report to QA is just an excuse to rob you of the money you earned. The crooks that do this should tell you before hiring what their policy is. Did you sign a contract? What does it say about that?
That was a while ago and they stopped docking pay.

Encourages guessing....totally wrong. This is a policy adopted by people who have never done the job.
Docking pay for errors is a very good idea, IMO. SM
Everyone is motivated by money.  It may be the only way to impress upon MTs how important quality is and make them pay attention to the feedback they get and implement the changes necessary.
Docking for errors should never happen! A certain Pennsylvania company does it, too. nm

EPT error
On a Friday I had my GYN to the urine test. At first it appeared negative, but after about 10 minutes up pops a second dot, faint at best. She took blood and told me that...I swear..."might be a little pregnant". You might want to go to a women's center where they will give you free tests.

this was dictated by ESL doctor:  unfortunately secondary to computer glitch, the patient was given Hytrin instead of Vytorin.


now i wonder, was this "computer glitch" bad enunciation that was mistranscribed or speech recognition gone bad maybe? 


ERROR message
I am trying to sign onto Docuscribe but it keeps giving me an error message that says "Editor failed to connect"  Can someone help?
simple MD error

They forgot to turn off the microphone before they said the words, OR they forgot to use the proper commands to direct you up to the location they wanted.

Had they done either, the words never would have appeared in the first place.


Error rates
Could anyone tell me how other service providers are dealing with error rates? Need to implement something but am unsure how to go about it. Thank you!

To me a gross error is something that changes the meaning of something. You didn't get that the MT was from California when he/she put "Ca" instead of CA????? Sheesh......give 'em a break.

This is a style error not a GROSS ERROR. Maybe you should put this in your advertisement "Please use AAMT BOS guidelines when filling out application and submitting employment letter". ROFLOL!!!!!!

Seriously though, I think if the MT has a good resume and performs well on the exam that he/she should be given a chance. Be clear that you expect proper style guidelines utilized. If they still want to do things their way (from the 1950s, LOL) then put them in your delete box :-)

Trial and error.
I check with former companies that I still have good relationships with. I check the job boards. I cold email "good" companies and asked about part time. I have 4 jobs at the moment, 3 transcription and I work at a vet 4 days a month for a couple hours just to take a break and do something I really enjoy.

That is not strictly an Extext error. It may actually be something to do with your Word software. If you are using any add-ins/macros with Word, they can cause this error also.
Major error?
That is not a major error. They are just two straggling headers that got left behind, an oversight at best but not major error category. What is wrong with these people? If they call that major what do they call a real major errors that could possibly affect the outcome of patient care? When you consider all the malpractice that goes on with direct patient care involving doctors and nurses, two straggling headers on a report is really very minor.
Macro help...error?!

I have set up several macros with my docs thank yous, closing and signatures.  They seemed to work fine whenever I set them up and then tried them out but now everytime I try and use one, it plugs in something that I earlier cut and pasted.  For instance, the thank you and closing are there but instead of the docs sig it is something wierd that I cut and pasted earlier.  I go into my macros and it looks right.  Anyone have any suggestions on how to fix this??  As always, thanks!  I don't know what I'd do without this website!!

I think they are actually about $5 per pill. Did you check? Was this an error or for real?
HIDWATCH Error.
Does anybody know how to fix this? I get this read error and my foot pedal quits working. The only way to get my foot pedal working again is to completely restart the computer. It's very annoying.

When I found this to be true (work available but blocked out), I called and questioned it. Perhaps she should do the same.
Not so funny error on my own MRI report
The MT apparently may have misunderstood the doctor when he dictated (English doc BTW) and typed "calcula" telangiectasia as opposed to capillary telangiectasia on the pons of the brain stem. This upset me a bit as I have some significant symptoms going on and my malformation is in a "high real estate area". I have had several MRIs in the past and they have been correct, except for minor typos (I know none of us are perfect). My real problem with this error is that it doesn't even make sense! Yes, the reports are being typed right at the local hospital. I have read several of my neurology reports also from the past that had many typos in them and it really makes you see just how important our job is, being accurate but also having a general knowledge of what you are typing about. When a new term phrase comes up, I take the time to check it out as many times I have found I have been wrong in what I thought it was and what was truly correct.

No, you're not in error at all. One of my medical sm
secretarial textbooks from 1967 listed all the states with the two capital letters. This was considered "new" then, shortly after the advent of zipcodes. Therefore, the two-capital-letter abbreviations have been "new" for 36 years!

AAMT BOS has an appendix listing all of these, with their state capital cities!



Hey, QA can be wrong, too. I know that I have made an error or 2 when I QA, but I
will always fess up to it, aplogize to the MT, thank her/him and take my whipping. That is what makes us all human.

Sometimes it is a matter of accounting error
when the time sheet is sent to payroll.  If they have you under certain budgets they may be trying to pay you from multiple departments and somewhere somehow the proper hours are not being sent to payroll.  Usually they will rectify the situation, but I can feel your pain.  This happened to me a lot when I was typing for several different departments.  If one manager forgot to put my hours in then I had to wait sometimes up to two weeks.  Hang in there!

I would suggest being very matter-of-fact about it, keep all emotion out of it, even you're really annoyed inside.
1 out of every 15 kids hurt because of error on MR
MSN has an article that 1 out of every 15 kids who are hospitalized are hurt or die because of prescription errors in the medical record!  Something happened to a child of Dennis Quaid and he and his wife have raised awareness of how important it is that correct dosages be placed on the medical record for patients.  I guess this would not just apply to the doctor saying it clearly and correctly, but to us typing it correctly.  Our job is not just typing, as you can see here.  What we put will definitely have an impact on the patient.  I was amazed when I saw how high the mortality rate is because of what is on the medical record. A good wake up call, I think!
I had problems with error messages
and the tech support at my company told me to run detect and repair in Word and that fixed it right up. Try that. Detect and repair is under the Help menu.
Common Error List

Does anyone have a common error list that a new MT could use? I have been doing transcription for about 6 years now and am starting to edit a new MT's work. I know I had a common error list with another company I worked with, but I tossed it when I ended my contract (I know, never throw away a resource! I am kicking myself now). I just was curious if someone had one and was willing to share :0) Thanks in advance!

You might try a yahoo search for "common error list in MT." I am sure you will come up with something that could be helpful.
double click the glossary that the error is in; sm
when it brings up the box, find the word/phrase you want to delete or change. once your cursor is on the line in the error, hit the delete button. FYI, you can also edit it rather than delete it.

Cuz you've never made a typing error?
you always type without an error?
That only flags spaces as an error. You cannot set Word to
automatically space for you. Replace is good, but watch out for values and things like Mr. when you do that.
