Help, what wouldyou do...sm

I am an IC for a small hospital.  I have had this account for over 10 years. I get paid by the minute of dictation and have only received one increase during that time which took some doing to get. Okay, here's my problem.  Recently, they (MR records) have been calling me to do normal op reports that the doctors do not dictate.   In the past, I have done them (at no charge) when the supervisor couldn't.  Now it has become at least once a week and they always need them that day and always call after I have finished for the day.  Would you charge for these reports and if so how would you charge?  Since I get paid per minute of dictation, should I charge per line for these reports or by report?

Thanks for any input.
