You got the first part right. You do go into the headings (CTRL-H). Then, you type it as you want it.

THEN - ALT-N (new).

At this point, I always highlight it and then do CTRL-E (expansions). Here, you name it. I always do mine like this hanes (for anesthesia) or hpid (for procedure in detail). Then, when I need it to be a heading, I type hanes and it comes as the red heading for anesthesia.
