If the transcribed letters were out in the open for all eyes to see, I would just ask "what are these doing here", and wait for the answer. Depending on what answer you receive, would depend on how you reply.
If the letters were not out in the open for all eyes to see and she just had them with her to deliver after work or something, I would do nothing and mind my own business. Everyone is entitled to make extra money, no matter what your feelings are, whether it be with the ex-partner or another physician's office.
This is my 2 cents - take it for what it is worth. Hope it helps.
