but then again I work for an ortho trauma doc. It never ceases to amaze me at how clumsy people are. I thought most people grew out of that "clumsy stage" after puberty. I am probably jinxing myself. Watch me fall and sprain my ankle on a pebble today. LOL

Know what I find interesting?
I'm impressed that you somehow were able to get this information. MQ offices are so... how should I put it nicely... not willing to divulge these kinds of things?

I mean, we (the noncherry-pickers) know this stuff goes on. I don't even bother to call and state my suspicions because I know they'll either not tell me the truth or your scenario will unfold. It's pretty much common sense and not worth the time... or the false hope.

I know you must be pretty bummed about about this, but at least you have a good comradery with someone in the office who tells you what's going on.

Having said all of the above, I just want to say I do love my office and know that I am getting dregs, but I do have an office that appreciates and compensates.

However, it does bug me that the MTs who won't or aren't given the chance (forced) to master the difficult doctors are going to lose in the long run. With VR and MTs who will and can do the difficult dictators (no-hope VRs), the rest will be phased out. They no longer will be needed and... that is the plan.

CHICAGO - A study of radiology reports generated by speech recognition software at a prestigious U.S. academic medical center found that even with fully trained system users the technology was introducing significant errors in almost one out of five signed, final reports.

"We structured a study where we could look at the types of errors and frequency of errors in attending dictated reports using voice recognition software," said Dr. Ronald Dolin, who is from the department of radiology at Thomas Jefferson University Hospital in Philadelphia.


Dolin and his team retrospectively reviewed radiology reports dictated and signed by attending radiologists from February to March 2006 at Thomas Jefferson. According to Dolin, all radiology reports at the institution were generated using PowerScribe 4.7 from Dictaphone/Nuance Communications of Burlington, MA, which had been implemented for 16 months prior to the start of the study.


A total of 395 reports, consisting of five to 10 consecutive reports from each of the 41 attending radiologists, were reviewed, according to the researchers. They categorized the dictation errors into 10 subtypes, such as missing word errors, wrong word errors, extra word errors, nonsense phrase, unclear meaning phrase, or abnormal phrase with meaning intact.


"Errors were classified as significant if they altered or obscured the meaning of the sentence in which they appeared," Dolan said.


A total of 239 errors were identified in 146 of 395 reviewed reports for an overall error rate in final reports of 37%, he reported. In addition, he noted that at least one error was identified in reports from 40 of the 41 attending radiologists at Thomas Jefferson.


The researchers found that missing or extra words that did not alter the meaning of a sentence constituted 113 of the 239 errors, or 47% of the total. This type of error was found in the reports of 33, or 81%, of the attending radiologists, Dolan said.


Other common errors reported by the scientists included the wrong word, which accounted for 21% of the total errors; typographical or grammatical errors accounted for 8.8% of the total; a nonsense phrase with unknown meaning showed up in 11% of the total errors; and an error in the dictation date presented in 2.9% of the erroneous reports.


Most of the speech recognition technology errors, 83%, did not alter the meaning of the report, Dolin noted.


However, the remaining 17% of the errors could have impacted patient care. Significant errors -- errors that could conceivably alter a patient outcome -- accounted for 40 of the 239 error total. He said that significant errors were found in 54% of the attending radiologist reports and that five radiologists had two or more significant errors.


Dolin acknowledged that he set the criteria for classification of significant errors and determined what was and was not a significant error. During a discussion of the research after the presentation, a few audience members vigorously expressed their belief that Dolin's standard for significance had not been inclusive enough.


Rather than being a condemnation of the technology, Dolin believes that his study can provide a method for Quality Assurance and continuing education in the vagaries of speech recognition software. A periodic audit of a relatively small number of radiology reports, such as five to 10 reports per radiologist, can identify significant voice recognition error patterns among the group and by individuals, and can assist in efforts to mitigate these problems, he said.

I find this interesting
and how much longer this will last. I know of 2 health care systems that are working very hard to use MTSOs in the USA and get away from Focus. Maybe it will rebound.
I find your post interesting. I am one of those that will not do this. sm
1. I teach at a local college. I teach MT classes, A&P and medical terminology. Guess what, they do not require it. Again they use my experience in lieu of it.
2. I know what position I currently hold. Again, does not need it and experience again is used in lieu of it.
3. I have 20 years of experience. Do I care if I have CMT behind my name? Really I don't. It will not make a difference.
4. I have worked in MT, QA, Management, etc. Again, did not need it do any of the above.

AAMT or AHDI as it is now callded.. they are there to sell a book to make money. They are selling a "title" to make money. However, this title makes little to no difference in the real world.

I have worked for companies that did pay their CMTs 0.5 cpl to 1 cpl line higher than people without it. Learn to negotiate is all I can say.

Your CMT certification does not keep jobs from going overseas and does not qualify you to have the knowledge to be an Editor of that type of work.

Anyone with 2 years' experience can take the test. Big deal. All companies test and I say at least half are bull. I have failed 1 test in 20 years, and then they hired me anyway because they felt there was something wrong (Thank you MTtest.com).

I have 20 years' experience. I am very good at what I do (and yes I will say this). I can also get the rate I want or tell them No Thank You.

I have worked with MTs from no experience to OMG experience. I have seen failure in both areas. The CMT would not have changed anything for either one.

A test is a test is a test. You might have a good day and pass it, or I guess you might not.

I just do not feel the need for someone to tell me that I know what I am doing.

I do find this an interesting comment.

They say, Have someone else transcribe it for you while you reap the lion's share of the profit.  Yes, jump right in and take advantage of others.  Seems that part may be true. 


This job is so easy you can do it with child sitting on your lap too, what a joke, what disrespect those scammers have for this job.

Then the OP goes on to say others are "negative"????? Do you find it offensive someone calls you a fool or you are acting foolish when you complain or joke about your work/manager/company?   When posters respond, they are unpleasantly surprised that this would produce negativity against the OP?

do radiology and then neurology and rheumatology......it all kind of links together in that way, at least for me 28 years ago is how it went....ortho to radiology to neurology and rheumatology and now today I can do it all........I especially love infectious dz, oncology, and I do not like OB/GYN nor eyes.

I have five years experience in both; work done is for the state of CA, although I do not live there. Every specialty has its own terminology, etc. It should not be too difficult a transition, but it could be challenging to make lines until you become familiar with it.

I've done ortho and acute care so far.

I like ortho the best, probably because I started out doing that. They have such a huge number of strangely-named signs and tests though that just slay me!

ER reports sound like they might be interesting. Seems like you'd hear it all doing those!

I have done ortho clinic in the past, just not 100% of the time.


I previously did multispcialty for many years which mostly consisted of family practice with an occational specialty thrown in to break the monotomy.


I would not say I am nervous about 100% ortho but I am "cautiously optimistic". Since it seems likeall I would be doing in the immediate future I was wondering if I should invest in the books etc.

Ortho
IMHO Ortho is the easiest of all unless it's urology.  An ortho book mighth make it easier for you...like Stedman's.  If you do ops think carpenter...hammer, screws, nails, etc.  You can use the Stedman's Word Book to look up say "screw" or "Pin" or "nail" to get the specific name.  I would highly recommend it.  Good luck to ya.

Need help with ortho term for PIP fracture

I'm working on an ortho eval for a patient with a PIP fracture.  Doctor has dictated an unfamiliar term (there's always that ONE WORD) for me.  He dictates, "X-ray examination today shows impaction at the base of the middle phalanx with approximately 70% joint surface involvement."  Here's the word I don't know (spelled phonetically--Sorry":  "A syphiant(??) portion of the joint surface is translated dorsally."


If someone could point me in the right direction for finding this word, I would really appreciate it!

When I tried Yasmin, it was going to be about $30/month on my insurance but I switched to Ortho-Novum, which is what I had used years ago for a long time anyway, and it is only $10/month. I know - it is ridiculous the way drug prices are.

My most worn Stedman's is the Ortho & Rehab.
The binding is totally beat and I actually have pages starting to fall out...second to that would be the Cardiology and Pulmonary words.  Congratulations on your new gig!

this is an ortho exam and he keeps repeating this throughout many of his dictations and I can't seem to find anything.  It sounds pretty clear that he is saying michelle flip sign but I just can't verify that anywhere.  Anyone ever hear of that?? Thanks so much for your help! 
Agreed--just bought my 2nd ortho book!

I actually bought it from somone on this board from the classified section who was getting out of transcription.  She sold me her fifth edition and I had either 3rd or 4th...and mine was absolutely beat--definitely my most used reference book.  She gave me a heck of a deal on that and that Surgical Equipment book...another lifesaver. 


So, if you're looking for books, buy from someone on here before buying new new! 


Also agree with one of those abbreviation books as well.  Another lifesaver if you've got an account where they want all abbreviations expanded or when you get to the final diagnosis part and you HAVE to expand them. 


I use dragon and I type ortho and ophthalmology.
It works much better for ortho.
