Rad exams
I have heard of these 100+ reports in an 8-hr day. I work on 2 different a/c's with 2 different methods of calculating lines. I do many MRI/CTs that are 40-50 lines or more. Interventionals are 100+ lines. 70-80 reports constitutes about 1700-1800 gross lines. No one on the a/c's I work on types more than 100 reports in a day - and these are top-notch MTs. I am waiting to work on an a/c where people type 150 reports a day - simply because I want to see what these reports are like.
Here's the scoop on the RMT and CMT exams...
From what I understand, the RMT is an entry level test which means nothing basically.  My employer doesn't pay anything for it.  However, we get a dollar an hour raise for passing the CMT, which is worth roughly 2000 dollars a year for me, so as useless as people like to say it is, would you take 2000 dollars or say "no thanks."  I know many employers pay nothing for it, in which case I wouldn't pursue it, but since mine does, I will probably try to pass it in the next year or so.  I hear the pass rate is abysmally low, which makes me wonder what is up with that. 
Exams Dictated But Not Done
I transcribe for a local multispecialty clinic and have seen many of my own reports. It just gripes my butt when the doctor dictates a standard physical exam and I know for a fact that he/she hasn't even touched me. Did not check my heart, lungs, abdomen, neuro, extremities, etc. The only things that are legit are the vital signs. It's like if I come in for a sore throat he looks at my throat but nothing else. My fibromyalgia clinic NP only talks to me - no exam at all - yet she dictates an exam, review of systems (that we didn't even talk about). She will say conjunctivae and lids are clear; PERLA, etc., and she hasn't even gotten within 6 feet of my eyes. This just doesn't seem right. And when you have labs or other tests, they NEVER call you back with the results. My PCP never suggests any preventive testing - I have to ask for it myself. I know they're not all like that but my eyes are opened much wider than they used to be. It's all about getting it done in under 15 minutes and on to the patient in the next room.

I love spaghetti but only served with a great salad with olive oil and some garlic bread. Can't have pasta alone.  And needless to say a glass of red wine.  No dessert, just extra spaghetti.

I don't think my son will have a problem. He wants to be a pharmacist. I've tried to talk him out of it and have tried to have him concentrate on his writing.


He has written several stories that have won awards.


Pharmacists are being held up at gunpoint for OxyContin. Scares the jeez out of me!


Oh well, you gotta let them choose what they want to do for a career unless they decide to deal drugs instead of dispense them legally.


5-10 of each a day with 2 others on acct doing 350-400 exams daily
Works out good if you are paid by exam which I hope you are.
We have three tiers, after 700 exams a week. 1.46/exam
We are happy with this, at least I am.

breast exams. We are required to have them for school before certain grades in our state, and never do they do breast or GYN exams on kids.

which are different than American med school grads. There was an article in The NY Times indicating that new regulations are being pushed that would require foreign med school graduates to take a two-part board exam to practice medicine here, with one part of the exam being English proficiency. Hmmm..that should be interesting. I don't think you have all of your facts straight.

I used to work for a large hospital that saw a lot of children for various horrible things.

The very last one I did was a 12yo boy who had hung himself while his mother was at work. She thought he was in school. The doctor cried, I cried (as I usually did) and I just couldn't handle that anymore. That was the longest report I swear. Short in lines but the length was almost an hour. The doc didn't pause the machine, she cried, horrible gut wrenching sobs, and I did too, right along with her.

their book. In fact, I love the BOS2. It is not a regular grammar book at all - there are hundreds of issues in it that are point specific to MTing and medical field. I have dozens of regular grammar books, too, for other purposes, and they are not similar at all. I purchased the BOS2 when out job interviewing for a few months. Every single employer wanted me to have my own company, and they all tested off of it. Several used it in their daily work, and I have found it priceless over all. I think its even on sale now, but $89 is not a lot for a professional tool that we should use if required, or if a newbie. MTs who don't like to buy books are not very wise in the big picture. Its like shooting yourself in the foot.
What's up with the CMT part A and part B exams?
I thought it was all one test.  How can a half a test last 6-1/2 hours?  If anyone knows, please share your info!  Thanks!

you can find great sales. I always keep an extra on hand, just in case.

I had wrist/elbow problems until I started using the 4000. It's a work and a wonder!

However, now I can't type on a straight keyboard to save my soul. It's a fair trade, I think.

Bytescribe wav player.  I've tried Express Scribe and Start Stop.  Express Scribe doesn't have good sound, and Start Stop was very buggy.  I've heard the older versions are better.  I bought mine in December 2005.  Wouldn't work right after several install/uninstalls.  Tech support couldn't help me.  I didn't have time to send it back before while it was eligible to return.


I play DSS files, and there's a trick to getting DSS files to play really really well on a Bytescribe.  Bytescribe wav player was made for wav files.  You can email me if you want to know how to do this or call Bytescribe.  They have another program you can use that converts DSS to Wav files.  It's already on your computer if you use the Wavplayer.


I would recommend Bytescribe if you're playing Wav files and Olympus DSS player pro if you're playing DSS files, although I've never tried DSS player pro but heard it's good.


I think it's worth the money to invest in a good player.  It will pay for itself over and over again in productivity and less mistakes because you can hear better.  It also pays to have a very good sound card on your computer, especially if you're on a laptop.

The Stedman's Pathology and Lab Medicine Words, 4th edition. You can find anything in there from bacteria to blood tests, etc. love it!

when I worked inhouse at a hospital a girl that worked in medical records was talking to her coworker who also did medical records and said loud enough that I would hear "a monkey could do transcription."  I guess insecurity plays a part in this kind of talk.  They must want their job to look more important.  Sorry, I cannot think of other reasons why people talk this way.

I really do love doing PT and OT except when they do their discharge summaries.


Why?  Because they have to list how many treatments, no-shows, and cancellations the patient had.


most of the time dictators have this information gathered before they dictate, but there's this one woman who constantly has us listen as she turns each page and counts up the totals.  So basically we hear "patient was seen... page turn 1, page turn 2, page turn 3, etc., until she gets to the total number of visits, (which can sometimes get upwards of 40!)


then she starts over turning pages and adding up the no-shows


and THEN she starts over turning pages and adding up the cancellations

My favorite is the doc who either
has a conversation with other docs and/or nurses when they should be dictating, or answering a phone call while dictating. I do VR and all that garbage goes into the report, and I have to delete all of it. Of course, some conversations can get quite interesting.

Ops always my favorite
from day 1. Started on ops as a beginner, still doing them and love them. Don't mind the other basic 4, but find all the meds and lab results annoying at times.

What are your favorite dictators?
I like the ones who right when they begin dictating, turn around and speak directly into their armpit.  Or how about the ones who carry on a complete conversation in English to the nurse next to them, then when they start dictating they sound like one of those auction guys who have had a few drinks?
My current favorite....
revises every sentence at least 3 times without fail. My first runner up dictates on a cell phone, drops every other call, and spells out every other word in case I don't know how to spell anything with more than 6 letters.

I have one guy, who really seems nice, thank's you each time at the end of the report, apologises if he has to correct something he said, long reports, BUT, he usually dictates at home, while cooking,bathing the kids and NOW, we must have a new dog because we are deciding to train it while dictating .


It's a verbat account so I really should have put in all the "sits, stays, good dog..would  have made a fortune.


BUT (again), if it were'nt for the fact that the ER he works in is over 140 miles away, I'd have been in the ER, requesting him, to treat me for my ruptured ear drum, when he must have forgot the doggie treats and this dog started barking, loud, blew my eardrums out, had to crank the volume wayyyyyyyyy down, could NOT hear him, except for the good dog's and sit, I'll play later


My two favorite specialties...
...radiology and op reports. In speaking with MTs, people who like one tend to like the other. I think it's because they both are fairly technical and have "flow." You're moving through a procedure or a test, and when you're done, your done.

I got into radiology without knowing anything about it and was the team leader within months. Just make sure you have a good reference or two, know your bones, muscles, and physiology, be willing to look up names of isotopes, machines, etc., and you'll do great!

My season is definitely fall.  I am up here in northern Maine and it is so pretty with the leaves changing, etc.  Of course then comes all the snow - and I don't even mind the cold or the snow but since I am getting to be "old and brittle" I am scared of the ice.  However, I really am looking forward to October.
