"Again" said randomly throughout sentences, starting them, ending them, a few times in the middle. Usually it made no sense and sometimes it was a real bind trying to figure out if it was safe to take one out or if that was violating verbatim rules.

"Again, the patient is 5 feet 3 inches, again her vitals are suchandsuch."

THAT MAKES NO SENSE. You can't say "Again" when it's the first time you're saying something!
