You how doctors always seem to have a pet word? They will use it until you are ready to smack them.

Subsequently. Said by an ESL at least once in every sentence and pronounced sub see QUEENT ly. He is also says Prolene mess potch and plook for mesh patch and plug.

Next...next this, next that, next, now I...

At this moment. Did you know you can start every sentence in a 2-page report with at this moment?

then
then
then

and THEN I screamed and was heard in the halls of every hospital in America.

Front-end speech is "talking;" back-end speech is flatulence. :-)

redundant, but (nm)

I see a pattern here
You are exactly in the position I was in UNTIL the bios (biological parents) decided they were too interested in their own lives to keep raising a child. My GD was literally dropped off on my door one day while I was working - with "everything" from home in a half-full pillow case. That was over 1-1/2 years ago. They now have bench warrents against them for failure to pay child support and my attorney has advised me to wait until they are in jail to have them terminate their parental rights so I can adopt the child.

Enjoy all that "spoiling" while it lasts, but start taking vitamins now because the web will probably spread further with you doing more and more in the parent role.

My "girl" is now 4 and I pay full time daycare for her as she will start school next year and I want on the same level as the other children her age when she starts. I don't have enough time in my day to work 8 hours, spend quality time with her, keep my house (I'm single with no other family to help), and teach her what she needs to be ready for school.
bloopers (forgive if they are redundant)

HOSPITAL CHART BLOOPERS

Actual writings from hospital charts:

1. The patient refused autopsy.
2. The patient has no previous history of suicides.
3. Patient has left white blood cells at another hospital.
4. She has no rigors or shaking chills, but her husband states she was very hot in bed last night.
5. Patient has chest pain if she lies on her left side for over a year.
6. On the second day the knee was better, and on the third day it disappeared.
7. The patient is tearful and crying constantly. She also appears to be depressed.
8. The patient has been depressed since she began seeing me in 1993.
9. Discharge status: Alive but without permission.
10. Healthy appearing decrepit 69-year old male, mentally alert but forgetful.
11. Patient had waffles for breakfast and anorexia for lunch.
12. She is numb from her toes down.
13. While in ER, she was examined, x-rated and sent home.
14. The skin was moist and dry.
15. Occasional, constant infrequent headaches.
16. Patient was alert and unresponsive.
17. Rectal examination revealed a normal size thyroid.
18. She stated that she had been constipated for most of her life, until she got a divorce.
19. I saw your patient today, who is still under our car for physical therapy.
20. Both breasts are equal and reactive to light and accommodation.
21. Examination of genitalia reveals that he is circus sized.
22. The lab test indicated abnormal lover function.
23. Skin: somewhat pale but present.
24. The pelvic exam will be done later on the floor.
25. Patient has two teenage children, but no other abnormalities.

I noticed a pattern last year..
I think last year he said it on the nights that they sang but not on the night that the results were given and they kicked somebody off. One night recently when the girls were singing, they all said it for him at the end of the show as a group. My husband and I say it when we Nextel each other. It is our signal that we are done talking instead of saying "over and out."
Some posters are newbies. If you do not want to answer redundant
you have a bad attitude for a message board poster. Some people don't mind reiterating regarding companies. If they do, they don't answer at all (unlike your soo freaking unnecessary post).
That's why I asked if it was a one-time thing or a pattern
A one-time slip-up is bad enough, but some people have a "serial cheating" personality that is unacceptable and definitely a deal-breaker.
Use main pattern, secondary backup patterns when first SM
isn't enough, and then deviate by choosing the "intuitive" choice for an abbreviation that pops into my brain, on the assumption that it'll be the one that pops in when I need the expansion. Usually it is.

I've always had the drop-down list turned off on my ShortHand program, so I can't speak to working that way. A lot of people clearly like it a lot.

However, when editing rather than transcribing, I question how well one can read ahead for corrections to pop down to and scan the dropdown list too. Fast-reading ahead and even making corrections while listening to the squeaky speeded-up dictation as it trails is how to make good money editing. One needs expansions a lot less when editing, of course, but they're still extremely handy. So if you choose to keep developing a system to work directly from as much as possible, rather than coming to depend on scanning a list, I think it should pay off nicely over the long term.

OTOH, since editing does need a lot fewer expansions overall, an Editor would come to memorize all the usual ones anyway and only have to stop and look at the list now and then, never in some documents, often in others. I'm not tempted to change my routine, tho.
VDI call-in system sounds much better than digital recorders and easier tracking with redundant hard
You can hear a pin drop on this system. The sound is that clear. The system has a redundant hard drive and auditing capabilities. It has the capability to do everything but transcribe it. The better the sound file, the faster you can transcribe. I purchased a call-in system that used digital recorders, and the sound was so bad that I ordered this one the same day. I wasted $1200 on that system. Get the analog that uses a VDI SuperStation or C-phone (the C-phone is more clear). Even though the technology is not as new as the digital recorders, in this case it is better. Good luck!
Thanks because that was really annoying ! nm

Oh yes. It is very annoying to say the least. sm
I take a lot of breaks during the day and go outside to get fresh air. I actually think the anxiety is what is causing the heart palpitations. I have a prescription for Xanax, which I cannot take and work, but I do use it at night before bed on a very infrequent basis.
yes really annoying
equipment, moaning patients, laughing and talking nurses, barking dogs, and children crying or trying to get mom or dad's attention (stop that, I'm dictating!) funniest one was an announcement over the loudspeaker about a pizza delivery to the nurse's station.
it's really annoying
happens quite a bit.   I do get the credit though I put in a note that says dictator did not complete report but I still get paid for what I typed.
that would be annoying....
guess there is job security after all.
very annoying
I have an RN who dictates I have a problem with this chart.  She also gives lab values and then says No, I don't like those lab values, just delete them. . It's very annoying and also, what does it mean? How can you not like the lab values. . . Just a little irritated this morning. . sorry to vent. .
annoying
It is just annoying to get your speed up on an account and get used to the dictator and then they go and quit giving you that work for a little while and then they want you to do it again. So wishy-washy. I don't get it. It seems like it would be easier on the company to put you somewhere and leave you.
annoying
Something that bugs me to no end is when dictators use the word "nor" constantly and incorrectly.  grrrrrr.....
Annoying!
I have one female provider who sounds like she is sucking on a lozenge the whole time. Worse than that, one doc clicks his pen over and over while I wait for him to say something (which is usually impossible to understand anyway).
ugh. baldwins are annoying now.
fat and bloated.
annoying dictator
Here's a good one; 7-1/2 minute dictation, first minute put on hold, then dictator dictates for two minutes, then says, Never Mind, Lets start this ALL OVER.  And they wonder why we are tense
Annoying dictator
If you are being paid by the line, I sure would dispute this! They have no idea, do they?
Do you ever get that annoying *bubble*

in your ear?  I have one in my left ear today and I can't get it to go away.  You know, when you talk and you can hear yourself really LOUD!  I have tried popping my ears....I tried irrigation....I thought I may have had water in it from my shower this morning, but nope.  It's driving me nuts and has made it very difficult to hear on that side today.  Does anyone have any tips on getting it to go away?

Annoying crickets sm
You know, I love summer with the sounds of birds and all that, but this cricket outside my window is driving me absolutely batty.  GRRR.
ok now I see it - extremely annoying. NM

Management again. SO annoying.....
Gaaa
Yes!! About 10 days for me. Very annoying.

annoying doctor
I have to say you gave me my first good laugh today. I have had doctors fall asleep and say, well, you know my routine, but just let me know if you cannot figure it out.
Oh golly, that must be frustrating and annoying.
maybe you can make a game for yourself of second-guessing the QA folks. It sounds like they are the kind of ill-informed people who would rather cling to their ignorance like grim death than open their minds and learn something useful. If you look at figuring out their foibles as a game, maybe it won't rankle so much.
So annoying to get a good debate going, and then it's gone.

How annoying. What is it with the flaky factor?

Not transcription related but I am trying to get volunteers to help with a big community theater project and I don't know how many times I get the "oh yeah I will definitely be there blah blah" and I end up with mostly no-shows!!! I mean I know it's volunteer but why lie? If they don't want to, just $%#^ SAY SO..  Can't imagine people doing that to a real job, what idiots.

Put annoying redundants in your expander...sm
and at the end of your pay period, count them and either aggravate yourself or LYAO all the way to the bank!
extremely annoying LOL I just did one today where the doc kept using the first name or the middle
None of which was spelled.  Wish they would make up their minds sometimes.  It would be funny (I was getting a little giggle out of it at the time I typed it) if it weren't so frustrasting at the same time.  Have a good one! :)
RE: I think they should send Paula home - she is so annoying........

New nominee for most annoying doctor today s/m
This guy is walking on the treadmill while he's dictating, so not only is he so scatter brained he doesn't know what he's talking about, I get to enjoy the heavy breathing.
these boards allow you to "backhand" annoying people so you don't take it out on your famil
and don't be arguing with me today. you know i'm right.
I have one who yawns a lot. Kind of annoying but I guess he's just tired! (nm)

Yeah, really! What an annoying, obnoxious, bottom-feeder she is!

Are you using Speech EXT or something else? nm
nm
speech
Transcription,Technology, and Support is supposed to be implementing this in the near future.
speech rec
It really does suck. No punctuation, wrong headings, puts in he instead of she or vice versa, lists numbered wrong, doctors names wrong. If you ask me, lets get back to what worked!!!!
speech rec
I absolutely hate speech rec. I see it as a way of utterly dumbing down the profession and paying us even less than we're already getting paid over time, like factory work. With our system (not Dictaphone but a major player), they have enabled essentially all the dictators (even those who are horrible), meaning that the proofreading is a killer, and it's not getting better, some dictations are absolute gobbelty-gook. These days, it ain't my wrists but my neck and left shoulder muscles that get the workout from the stress of how hard I have to concentrate. We've been told that the physicians must never see what the report would look like if we weren't charged with making absolutely certain it's picture perfect, as though we'd typed it! And yeah, I'd just as soon delete the whole durned report on some of these and just transcribe it. To heck with production, which is the constant mantra where I work. Again, I absolutely HATE it and pray it will somehow collapse, but I don't see it happening, not with jobs being so hard to come by.

I can foresee where speech rec could be "perfected" to the extent that there will come a time when an intelligent person could conceivably just sit down and read through these reports and actually learn medical terminology and the whole process by just doing speech rec. Again, like factory work. Think about it.
On speech rec
The company I work for pays 4 cents a line for speech recognition and the minimum for speech rec is 11,000 a pay period. That comes out to $5.50 an hour. I believe the minimum wage in Texas is $6, so I do not make minimum wage doing only SR. To get minimum wage I have to do 12,000 lines a pay period. Actually I do more than that because of the cost of living!
MQs speech recognition.
:
speech recognition
I worked with speech recognition at my last job and I didn't like it.  It took longer and alot of mistakes came through.  You were editing, but I found I could do a better and faster job typing it myself.  Everyone's production went down and error rates went up because you become complacent and when you are reading along with the words playing in your ears your eyes have a tendency to overlook a lot of little things that should be edited.  I did not find it productive or helpful just a mess.
Speech recognition
I am on speech recognition as it comes up. Don't have it all the time and it is on the DQS platform. I enjoy the break from typing but can be slow as you really have to listen to every word and make the corrections where necessary. The fastest typists just erase the entire report and retype. I just listen and make corrections. I have not had a pay reduction as of yet since this is not all I do. I am still paid my regular per line rate for SR but rumor has it that this will change but this is only a rumor, nothing in writing. If you do not like editing, can always tell them that you just want to type and not edit. I am on high speed internet also. Hope this helps.
Ah, a figure of speech. (sm)
Like a burden or baggage from a person that will not take the time to learn or understand?  One who doesn't want to fix it or get off his/her duff to understand?  One who generally doesn't care?  Anyway, I thought my original post was perfectly understandable and if they didn't understand it that is not my problem!
