Not an ER doc, but...sm

Last time my dad was in the hospital I actually had to type his pulmonary consult, and oh my goodness, it was detailed.  Curiously, when Dad got home, he told me that that particular doctor didn't even touch him.  So how did this doc decide his abdomen was soft or that he had no pedal edema?  (Although it is possible Dad just doesn't remember being touched, he's getting to that point.)  I have been to the ER and typed my own report, and what the doctor said and what he actually did are not the same.  I think it's just a habit of saying the same thing.  Haven't you ever had a report where, for instance, the chief complaint was icterus and then on the physical the doc said "anicteric"?  I have.  They just go so fast when they talk and don't think.  JMO along with fact.
