Just last week I was typing an H&H for cataract surgery and the doctor would say right eye one time and the left eye the next. The patient already had one eye done and he was no better at that, kept getting the eyes mixed up.  I typed the report and made a note to the QA explaining what the problem was.  The next day I went back into the report and the report was sent on just the way it was typed.  He made no changes whatsoever, so I contacted my sup and explained the situation to her and said I did not want to get blamed for any errors, I typed the report the way it was dictated and the QA did not correct it and asked her to please followup and see that the corrections are made.    I was supposed to hear back but I have not heard anything back.  But I have a copy of my e-mail to my sup.
