QA vs QC

Only charts that have QA markers or 100% MTs go through QA.  Some clients require all of their charts go through QC to have ADT checked and verify the match, etc.  Some clients require their addendums be added to the original chart, again a QC/clerical function and not a QA function.  So, not every chart transcribed goes through QC and even less through QA.  Therefore, many of charts never stop between the MT uploading and it being delivered to the client.
