Question for ICs

What do you do when you have foreign speaking doctors with very heavy accents who dictate so fast it's not humanly possible to understand what they're dictating,  and whom you have asked time and time again to PLEASE SLOW DOWN! 

Do you guess so that you won't have to leave blanks or do you leave blanks? 

I leave blanks, but I'm getting so tired of doing this AND their work!  I feel like it's a waste of their time and mine!
