If the statement within the punctuation is a complete quoted statement, then a period is required at the end of the statement and another one after the parentheses.  (i.e.:  The patient stated "I cannot stand this headache anymore.".)


If the statement within the punctuation is not a complete quoted statement it does not need a period.  (i.e.:  The patient stated her headache over the last few days felt "weird".)
