Working inhouse at my first transcription job in 2002 with no prior transcription training, nobody there ever heard of BOS and there were no ESL doctors. I learned lots of bad habits because it was how they wanted it done, and I had nothing to compare it with, until I went to the next job. The only thing that mattered was what the docs wanted. There was no QA, if you had a blank you went and found the doc or asked a coworker.
