 I was the senior transcriptionist/supervisor for a local behavioral health center for years.  I would not, I repeat, would not, transcribe a report on a family member or friend.  Their personal struggles was none of my business.  I would have another MT transcribe it.  They did the same, if a report on a friend or family member came across their desk.  I do not think it is ethical at all to be transcribing a family member or friend's reports.  I personally would not want to be at a family reunion and know all about Cousin Mike's personal struggles.  I would not want them to know mine either.  If I did, I would talk to them, not a counselor.  Just my 2 cents worth on the issue.
