 I know that they became "dangerous abbreviations" at least that I know about three years ago.  They have also added qhs to the list of items considering qd was taken away for daily and hours of sleep was too long they used nightly.  Now, do not know who was the ones who decided this but know our Formulary has definitely used them for the last three years.  Believe if falls under, "if in doubt spell out" ruling
